WHAT IS YOUR NEW PATIENT PROCEDURE?

If you have insurance please call us and we will schedule you with a physician. 

If you do not have insurance but want to get insurance, please contact us and we will assist you.

If you are paying "out of pocket" just call us and schedule an appointment.

DO YOU DO SCHOOL PHYSICALS?

Yes, we do. Your child's immunization records will be readily accessible on the New York Citywide Immunization Registry (CIR).

DO YOU DO JOB PHYSICALS?

Yes, we do at no extra charge.

DO YOU COMPLETE DOT FORMS?

Yes, we do, on Mondays and Wednesdays.

DO YOU TAKE WALK-INS?

Yes, we have been taking walk-ins for over 50 years, way before "urgi-care center" became a thing. We do encourage an appointment as appointments are taken as a priority. We will try our best to accommodate all patients that walk in. If you arrive at a minimum of 1 hour before closing time, we will do our best to see you.

WHAT CAN I DO TO SPEED UP THE CHECK-IN PROCESS?

We encourage all patients to make sure to bring their current insurance card (if applicable), and another form of state or federal ID. The front desk will require your updated address and telephone number as well. If you are a Workman's Compensation (WC) or No-Fault (NF) patient, please identify that to the Front Desk immediately, as you will be fast-tracked to the appropriate Doctor and/or Caseworker. We call all patients the day before to confirm their appointment, at that time, if you need to reschedule, please let us know. If we do not have your current phone number, understandably, we cannot contact you, that is why it is important to update this information every time you come into the Medical Office.
